Incidents often occur on school property and some result in an injury to an employee. The School District carries Workers’ Compensation insurance on all staff members which covers such injuries to employees that occur as a result of their employment. In order to ensure the proper reporting of all incidents, the following procedures have been established. Failure to follow such procedures could result denial of claim.
1) Any "on the job" incident must be reported to the Principal's Office or other Department Administrator immediately. Please refer any minor injury to the school nurse for basic first aid and evaluation. For life-threatening injury, please call 911 for immediate assistance.
2) The employee’s immediate supervisor is to complete the District 25 Incident Report form. This form must be returned to the Business Office, Attention: Supervisor of Business Services, within 24 hours of the injury.
