New Student Registration Information
If you wish to register your child during the summer when elementary school offices are closed, registration packets can be obtained at the feeder middle schools. If you are not sure of which middle school you should visit, contact the district office (847-758-4900). The middle school secretaries give new parents a registration packet and record the names and grades of the new registrars. The middle school secretaries are best qualified to answer school related questions to new families. The offices of the elementary schools re-open in early August, and the elementary secretaries will then contact the parents of new registrars.
Registration forms and requirements:
In order to register for school, the first thing the district requires is proof of residency. Parents must fill out a CERTIFICATE OF RESIDENCE, and supply us with two forms of identification proving residency. We accept lease, mortgage, any 2 utility bills, public aid card, or a property tax bill. We DO NOT accept a driver's license. If a family just moved in and cannot provide us with proof of residency, we cannot continue the registration process.
The original birth certificate, from the Bureau of Vital Statistics, NOT hospital footprints or a baptismal certificate, must also be provided.
Assuming prove of residency, the rest of the packet includes:
District Registration form (fill out, sign)
Emergency Form (fill out, sign)
Home Language Survey (fill out, sign)
Consent of Release of Information (fill out with name and address of
previous school, sign)
State of Illinois Student Transfer Form (Must be filled out by previous
school)
Elementary Fee Sheet (pay fees)
Bus/Transportation Application
Medical forms, depending on the grade level - Physical, Immunizations,
Dental & Vision
If applicable, copy of student I.E.P.
PTA membership/school directory forms (optional)

Illinois State Report Card (if they ask)
Children At Play Program
If there is a question of who has custody of the child, we require a copy of the court order stating who has custody. It must be stamped and dated by the Circuit Court.
