• Incidents often occur on school property and some result in an injury to an employee. The School District carries Workers’ Compensation insurance on all staff members which covers such injuries to employees that occur as a result of their employment. In order to ensure the proper reporting of all incidents, the following procedures have been established. Failure to follow such procedures could result denial of claim.


  • 1) Any "on the job" incident must be reported to the Principal's Office or other Department Administrator immediately. Please refer any minor injury to the school nurse for basic first aid and evaluation. For life-threatening injury, please call 911 for immediate assistance.


    2)  The District 25 Incident Report form must be completed, signed by an administrator, and submitted to the Business Office within 24 hours of the incident. 
In addition, an Administrator must complete and submit the IPRF Supervisor's Investigation Report.


    3)  If outside medical attention is needed, please refer the employee to their own doctor or one of the Northwest Community Hospital Occupation Health facilities:

    The employee is to instruct the doctor/hospital that the injury is being claimed under worker’s compensation insurance and all bills should be sent to:
