Welcome to Student Services!
Welcome to the Student Services department via www.sd25.org. We are here to provide support and services to the students and families of District 25. Please review our department's website for any information or forms you might need and reach out to our department directly if you need additional information.
The vision of the Department of Student Services is to... Collaborate...Support...Empower for Excellence
The mission of the Department of Student Services is to meet the unique needs of each student by providing high quality specialized services in collaboration with staff, families, and the community.
The purpose of the Department of Student Services is to maximize student' potential as members of their community through the provision of an exceptional education.
Every year, District 25 partners with NSSEO to offer a survey to parents with students receiving special education services in our district. This survey provides valuable feedback on programs, support systems, and communication. In addition to helping District 25 improve the programs that we provide, it also helps NSSEO tailor family-based supports. We will reach out when the 2020-2021 school year's survey becomes live.
Thank you for taking the time to give us your feedback!
Transitioning to Middle School Presentation

For those that couldn't attend, the presentation was recorded:
Special Parent Messages or Information
School personnel who provide related services to students are required to maintain written logs that contain the service provided, the date and number of minutes administered. These related service logs must be provided to parents/guardians during the student’s annual review IEP meeting and also anytime upon request. These service logs are considered part of a student’s temporary record.
